This is not an order form, it is a registry form. Each bridesmaid must place an individual order with Park Avenue Bridals. Once all individual orders and payments are received from a bridal party (payment in full is required), we will confirm the order with the manufacturer. Before your order is finalized, if any information differs from that noted on this registry, please notify us immediately. Once an order is placed, no changes can be made to the order and there are no cancellations.

*Dye lots may vary from the manufacturer swatch cards, colors online and sample dresses. Dye lots are especially NOT guaranteed by the manufacturer when ordering different style numbers, gowns consisting of more than one fabric, or gowns ordered at a later date.
*There is a $15.surcharge per dress when ordering different style numbers.
*Customer must request extra length if desired (at an additional charge, if available from the manufacturer). Most gown hem lengths fit approximately 5'7" in bare feet.
*The final decision on size selection is the sole responsibility of the customer. Park Avenue Bridals will not assume any responsibility for the size selected by the customer. We cannot recommend a size nor suggest a size for any customer. Please keep in mind that most bridesmaid dress sizes run small.
*Gowns are not custom-made to measurements, and almost all dresses will require some type of alterations.
*Your delivery can take up to 24 weeks (from the time the LAST bridesmaid orders her dress), so please finalize your order in a timely manner. Please call us in 16-24 weeks (after you place your order) to check the status of your order.
*Park Avenue Bridals or its suppliers performance under these Terms and Conditions of use are subject to interruption and delay due to causes beyond their reasonable control, such as acts of God, acts of any government, war or other hostility, civil disorder, the elements, fire, explosion, power failure, equipment failure, viruses, pandemics, industrial or labor dispute, inability to obtain necessary supplies and the like.
*If available, Park Avenue Bridals can order fabric at an additional charge. Delivery for fabric may run 8-12 weeks. Color dyelot of fabric is NOT guaranteed to match color of dresses.
*Park Avenue Bridals does NOT offer alterations.
*Shipping charges are additional. Please call the salon for pricing.
*Dresses not being shipped must be picked up from Park Avenue Bridals within 1 week of arrival in the salon.
